Create New Student Profile

PLEASE READ CAREFULLY BEFORE PROCEEDING: The following information is required to become a career credit student.

APPLICANT EMAIL

The profile must be unique to the applicant, and a unique email account is required to create a profile and register for a class. Your email becomes your username. Additionally, all important class information, such as confirmations, reminders, updates, and cancellations will be sent to this email. If you do not have an email account, please contact our office at fastforward@patrickhenry.edu or 276.656.0260 for assistance.

APPLICANT NAME

Enter your full legal name, as it appears on your current legal documents such as driver's license or social security card. Capitalize the first letter of your first, middle, and last name. Name provided will appear on transcript, certificate, certification, and licensure documents.

SOCIAL SECURITY NUMBER

Not entering your social security number will hinder your financial assistance and/or refund processes. Additionally, some state licensing agencies require social security numbers as part of the application / renewal verification process.

UP TO DATE INFORMATION

Once this information is entered, it is important to keep it as current as possible. This information is used for verifying your identity and contact information each time you register for a class.
